Nancy Swayze, MD is a graduate of the Univ of Rochester School of Medicine and Dentistry. She is board-certified in Internal Medicine and Hospice and Palliative Care and serves as the Chief of Skilled Rehab. She sees patients in nursing homes, either long term residents or those needing skilled rehab after an acute illness. Dr. Swayze approaches each patient's needs individually and has the knowledge, skills and expertise to provide great care. In partnership with our in-practice geriatric teams, nursing home nurses and staff, skilled rehab teams, and hospice personnel, she leads a team of experts managing the physical, social and emotional effects of aging.
