• Breast Augmentation

  Breast augmentation (mammaplasty) is one of the most popular cosmetic surgery procedures in the United States. Hundreds of thousands of women each year have this surgery to improve their appearance. Breast implants can be used to increase your breast size, restore lost volume, create greater symmetry for uneven breasts and reshape your bust line. Candidate Profile If you’re 18 years or older and feel self conscious about your breasts being too small, uneven or lacking in firmness, you may benefit from breast augmentation. Ideally, you should be in good overall health and have realistic expectations about the benefits and potential complications of plastic surgery. You may wish to stabilize your weight before having this operation; substantial weight loss after surgery may affect the appearance of your implants. Heavy smokers and patients with health conditions that increase the risk of side effects during or after surgery may not be the best candidates for this operation. Patient Consultation Overview During the consultation, your cosmetic surgeon will discuss your options and expectations including: The type of implant you want (saline may require a smaller incision, but many patients prefer the more natural feel of silicone) The volume of each implant (measured in cc’s) required to achieve your desired cup size The shape of the implants (round or teardrop) based on your desired breast profile Implant placement (beneath the existing breast tissue or under the pectoral muscle) How the implant will be inserted (under the breast, through the armpit or around the lower edge of the areola) If the placement of your nipples will be adjusted and whether a breast lift (mastopexy) may be needed to correct drooping Your plastic surgeon will also explore your full medical history and the potential risks and side effects of surgery. You can discuss breast augmentation costs with the patient coordinator along with payment plan options. Breast augmentation prices can vary substantially based on a number of factors unique to each patient; the total cost may be more or less than the prices you have seen on other cosmetic surgery websites. The Procedure This operation is done under general anesthesia or intravenous (IV) sedation to ensure your comfort and safety. The procedure may take 1 to 3 hours. The surgeon makes an incision, moves tissue aside to create a pocket and inserts the breast implant. Silicone implants are inserted pre-filled while some saline implants may have their volume adjusted after insertion. After implant placement, the incision is closed with sutures. A drainage tube may be placed to help manage fluid accumulation. This tube will be removed later. Most patients return home the same day. Recovery Following all post-procedure instructions is critical for optimal breast augmentation recovery. If you have a job that requires a lot of sitting, you may be able to return to work in less than a week with your plastic surgeon’s approval. Physical activity involving moderate lifting, pushing and pulling will be limited for some time after surgery. Resuming your normal exercise regimen may not be advised until 1 to 2 months after the operation. You should be able to return to light everyday activity such as walking, sitting and standing within a day or two after their breast enhancement procedure. You may begin showering again 3 to 7 days after surgery once the bandages are removed and your surgical incisions have begun to heal. Stitches are typically removed after about a week. Pain, breast sensitivity, skin tightness, discoloration, itching and swelling are normal side effects of this surgery. These symptoms should resolve with time. Breast augmentation scars will remain, but typically fade to become less noticeable. Potential Complications This operation carries many of the same complication risks as other types of plastic surgery. Complications that are unique to breast augmentation include: Capsular contracture (development of hard, fibrous scar tissue around the implant) Rippling or unevenness of the skin over the implant Implant shifting out of position Implant rupture and deflation A dissatisfactory outcome that may call for revision Changes in breast and nipple sensation (which may be permanent) Difficulty obtaining clear mammogram images Inability to breastfeed Results Many patients report greater satisfaction with their appearance and more self confidence after receiving breast implants. You should choose a plastic surgeon with appropriate credentials and a substantial amount of experience to increase the likelihood of a satisfactory result.

• Breast Lift

  Breast Lift Details A breast lift, known as mastopexy, raises and firms the breasts by removing excess skin and tightening the surrounding tissue to reshape and support the new breast contour. It can also help in cases where the areola becomes enlarged over time or where a patient may have lost a significant amount of breast volume. In such cases, breast implants in conjunction with a breast lift can achieve breast enlargement at the same time as the shape and position of the breasts are enhanced. Although local anesthesia may be used, general anesthesia is generally administered. With general anesthesia, you will be asleep. With local anesthesia, the selected area will be numbed. Description of the procedure. A common method of lifting the breasts involves three incisions. One incision is made around the areola. Another runs vertically from the bottom edge of the areola to the crease underneath the breast. The third incision is horizontal beneath the breast and follows the natural curve of the breast crease. After your plastic surgeon has removed excess breast skin, the nipple and areola are shifted to a higher position. The areola, which in a sagging breast may have been stretched, can be reduced in size. Skin that was formerly located above the areola is brought down and together beneath it to reshape the breast. The nipples and areolas remain attached to underlying tissue, which usually preserves sensation and the ability to breastfeed. Variations to breast-lifting techniques. There are many variations to the design of the incisions for breast lift surgery. The size and shape of your breasts, size of your areolas and the extent of sagging are factors that will help your cosmetic surgeon determine the best technique for you. In some instances, it may be possible to avoid the horizontal incision beneath the breast. Sometimes a technique may be used that avoids this horizontal incision, as well as the vertical incision that runs from the bottom edge of the areola to the breast crease. If you are a good candidate for a modified technique, your doctor will discuss this with you. If you and your breast enhancement surgeon have decided that it is desirable to have a breast enlargement at the same time, this will require breast implants. He/she will review the necessary information with you and may provide you with a brochure on breast augmentation. Closing the incisions. After your breasts are reshaped and excess skin is removed, the remaining skin is tightened as the incisions are closed. Some incision lines resulting from breast lift are concealed in the natural breast contours; however, others are visible on the breast surface. Incision lines are permanent, but in most cases will fade significantly over time. Sutures are layered deep within the breast tissue to create and support the newly shaped breasts. Sutures, skin adhesives and/or surgical tape may be used to close the skin. How long will it take? About one to three hours. How much will it hurt? Anesthesia prevents pain during surgery. You may have some bruising and tenderness of the breasts for several weeks after your cosmetic surgery. Talk to your plastic surgeon about medicine to help manage the pain.

• Breast Reduction

  Breast reduction surgery (reduction mammaplasty) is a plastic surgery procedure that decreases the size of the breast by removing unwanted fat, glandular tissue, and skin. Women with very large breasts are most likely to request this procedure. However, a less extensive version of breast reduction called “gynecomastia surgery” is becoming increasingly popular among men who have excess breast tissue. Candidate Profile If you’re a woman with breasts that are disproportionately large or asymmetrical, you may be a candidate for this surgery. Ideally, you should be in good overall health and have realistic expectations about the benefits and risks associated with cosmetic surgery. If you’re not at or near your ideal weight, your surgeon may recommend a weight loss program prior to this operation to minimize the amount of fatty tissue that must be surgically removed. Patients who smoke or have a health condition that could increase the risk of surgical side effects may not be the best candidates for this procedure. Patient Consultation Overview Your plastic surgeon will need to know your treatment goals. For example, you might be seeking this procedure to relieve one or more of the following symptoms: Back, neck and shoulder pain caused by the weight of your breasts Difficulty maintaining correct posture due to being “front heavy” Chronic yeast infections or other rashes under your breasts Breasts that are not the same cup size Drooping caused by age or as a result of pregnancy/breastfeeding Persistent irritation or grooves caused by bra straps Your cosmetic surgeon will review your entire medical history and walk you through the potential risks and side effects of the operation. In some cases, reduction mammaplasty is considered medically necessary rather than cosmetic. You’ll need to discuss your case with your surgeon and your insurance provider to find out whether any of your breast reduction costs may be covered by insurance. Breast reduction prices can vary depending on the extent of the surgery, whether both breasts will be reduced, and whether additional procedures such as a breast lift will be done at the same time. The Breast Reduction Procedure This procedure may be done under either general or local anesthesia depending on the surgeon’s assessment of the nature of the operation and your specific medical profile. This procedure may take 2 to 4 hours. The surgeon typically makes an incision around the areola, down to where the breast meets the rib cage, and along the breast crease. This is where your breast reduction scars will be. The surgeon will remove skin, fat, and breast tissue as needed to reduce and reshape the breast. Liposuction may be used for fat removal. The nipple and areola may need to be repositioned to provide a normal appearance. The incisions are sutured closed and small tubes may be placed temporarily to drain excess fluid. You may be fitted with a special surgical bra or compression bandages to aid healing. Breast Reduction Recovery Some patients go home the same day while others must remain in the hospital for several days. Drains are removed after about three days and sutures are removed after 7 to 10 days. Tenderness, swelling, and bruising are normal side effects that should resolve over time. Your surgeon will let you know when you can resume regular activities. If your breast reduction recovery goes well, this may be just a few weeks. Potential Complications from Breast Reduction Surgery This operation carries many of the same complication risks as other types of plastic surgery. Potential complications that are unique to breast reduction include: Asymmetrical or unsatisfactory results requiring revision Changes in breast and nipple sensation (which may be permanent) Tissue death of any portion of the breast due to insufficient blood supply Fluid- or blood-filled cysts (hard lumps) that develop during the healing process Possible inability to breastfeed Limited arm and/or shoulder movement after surgery Breast Reduction Results Many patients who have breast reduction surgery experience significant, ongoing relief from backache and posture problems. • Male Breast Reduction

  What Is Gynecomastia? Gynecomastia is enlargement of the breasts in men. It is relatively common, occurring in about one-third of men. About 65 percent of boys develop some degree of breast enlargement during puberty. This is normal and usually goes away by age 18. The breast tissue is firm in men with gynecomastia. In contrast, the breast tissue is soft in men with fat on their chests, hence looking like enlarged breasts (fatty enlargement of the breasts). Causes | Risk factors | Symptoms | Diagnosis | Prevention | Resources | References Causes Of Gynecomastia Men normally produce much more male hormones (androgens) than female hormones (estrogens). Gynecomastia is caused by an imbalance in the female and male hormones. The hormone imbalance can be caused by: Aging Testosterone deficiency Certain medications, such as digoxin (a heart medication), spironolactone (a diuretic), cimetidine (a medication for stomach conditions) and many others Anabolic steroids used to enhance athletic performance in sports Marijuana use Liver or kidney failure Chronic kidney disease Hyperthyroidism (overactive thyroid gland) Tumors of the testicles, lung, stomach, liver, kidney or pituitary gland Gynecomastia Risk Factors A risk factor is something that increases your chance of getting a disease or condition. By definition, only men can get gynecomastia. Risk factors: Age: over 50 Obesity Excess alcohol consumption leading to liver cirrhosis Chronic liver or kidney disease Presence of a condition or medication that decreases androgen or estrogen production Family history Symptoms Of Gynecomastia Symptoms of gynecomastia include: Enlargement of the breasts with firm tissue, usually on both sides Tenderness How Is Gynecomastia Diagnosed? Your doctor will ask about your symptoms and medical history, and perform a physical exam. You may need to be referred to a doctor who specializes in hormone disorders (an endocrinologist). Your doctor will be especially interested in other symptoms you have had and your use of medications. He/she will focus on your weight, breast exam, testicular exam and any other signs of a hormone problem. Diagnostic tests may include: Blood sample — to check the function of your liver, thyroid and kidneys, as well as other hormone levels Ultrasound — a test that uses sound waves to examine the breasts CT scan — a type of x-ray that uses a computer to make pictures of the breasts Biopsy — if the diagnosis or cause remains unclear, a sample of breast tissue may be removed and sent to a pathology laboratory for evaluation Treatment For Enlarged Male Breasts Usually, treatment for gynecomastia is not needed. However, it is important to find and treat the underlying cause of the gynecomastia. For example, if you are taking a medication that causes gynecomastia, your doctor will ask you to discontinue it or change to a different medication if possible. It is particularly important to exclude tumors as a cause of gynecomastia, and, if one is found to be present, to treat it. If treatment of gynecomastia is needed, medications are sometimes used, though they can produce unwanted side effects. On rare occasions, gynecomastia surgery by a plastic surgeon is used to remove breast tissue. Can Gynecomastia Be Prevented? Prevention of gynecomastia requires avoiding known risk factors. Thus, avoiding excessive alcohol consumption, avoiding steroids and refraining from marijuana use will prevent gynecomastia from those sources. Some factors that may affect results include: Age — Older patients may not see the same results as younger patients because their skin is less elastic Experience of the doctor — Plastic surgeons with more experience tend to have fewer problems Some factors that may increase the risk of complications from lipo include: Diabetes Smoking Severe heart or lung disease Taking certain medicines Poor blood circulation Recent surgery in the area to be suctioned Having a large area suctioned, or large amounts of fat removed What to expect from Liposuction Prior to procedure. Your plastic surgeon will likely: Evaluate you as a candidate for liposuction surgery: Ask about your medical history, illnesses, medicines, drug allergies and previous surgeries You may be asked to stop taking some medicines (including herbal supplements) up to one week before the procedure like: Aspirin or other anti-inflammatory drugs Blood thinners, such as clopidogrel (Plavix) or warfarin (Coumadin) Discuss previous weight losses/gains and how they affected your body Have you identify the areas you would like to have suctioned Test your skin elasticity (ability of the skin to stretch and return to normal) Estimate the amount of fat needed to be removed for best results Discuss your ideal surgical outcome and body image versus realistic expectations Determine your emotional stability (after plastic surgery, some patients tend to become depressed temporarily) Discuss the different types of liposuction available Prepare you for the procedure by: Discussing surgical techniques and anesthesia options Giving you instructions Addressing your questions and concerns Leading up to your liposuction cosmetic procedure: Refrain from smoking and drinking alcoholic beverages Follow your doctor’s instructions regarding diet Arrange for a ride and for help at home The night before, eat a light meal; do not eat or drink anything after midnight If advised by your cosmetic surgeon, take a shower in the morning or the night before the procedure; you may need to use special antibacterial soap Anesthesia. There are two anesthesia options for liposuction surgery. Your doctor will help you to decide which is best for you. Local anesthesia — This numbs the area. You will be awake during the procedure. General anesthesia — You will be asleep during the procedure. Description of procedure. After anesthesia has taken effect, a special fluid containing saline (salt water), additional anesthetic, adrenalin (to minimize bleeding) and bicarbonate (to minimize pain from injection) will be injected into the fatty areas. One of the following three extraction techniques will be used: Wet technique — The amount of fluid injected is less than the amount of fat to be removed. Super wet technique — The amount of fluid injected is equal to the amount of fat removed. Tumescent technique — Three times as much fluid is injected to the body as fat removed. After the fluid is added, a small incision will be made near the area to be suctioned. In traditional liposuction, the lipo plastic surgeon will use an instrument called a cannula to suction the fat. A cannula is a hollow tube, like a drinking straw. A vacuum pressure unit, which is attached by a hose to the cannula, will provide the suction for the procedure. Once fat has been removed, the incisions may be sutured closed or left open to drain. The following are different types of liposuction surgery available: Power-assisted liposuction — This involves the use of a vibrating cannula that disrupts the fat cells prior to their removal. The technique is especially useful in areas where the fat is more difficult to remove or in areas previously liposuctioned. Ultrasound-assisted lipoplasty (UAL) — This involves the use of ultrasound energy provided via a probe under the skin. The fat cells are disrupted and the fat is then removed by traditional liposuction. There may be a reduction in bleeding and swelling and an increase in skin tightening with this procedure.

• Thigh/Buttock Lift

  A thigh and buttock lift reshapes the thighs and buttocks by reducing excess skin, and in some cases fat, resulting in smoother skin and better-proportioned contours of the thighs and lower body. Reasons for procedure | What to expect | Call your doctor | Resources Reasons for procedure If fitness and weight loss efforts have not achieved your goals for a body that is firmer, more youthful looking and more proportionate to your overall body image, a surgical thigh or buttock lift may be right for you. A thigh and buttock lift tightens loose, sagging skin in the buttocks, thighs and hips, and improves the appearance of dimpled skin. What to expect Prior to procedure. Prior to butt lift surgery, your doctor may ask you to: Get lab testing or a medical evaluation Take certain medications or adjust your current medications Stop smoking well in advance of surgery Avoid taking aspirin, anti-inflammatory drugs and herbal supplements as they can increase bleeding Anesthesia. Thigh and buttock lift cosmetic surgery is typically performed under general anesthesia, and you will be put to sleep with an intravenous sedative. After you are asleep, a breathing tube will be placed in your mouth to ensure that the airway is secure during the position changes necessary during the procedure. Description of the procedure. Incision patterns vary based on the area or areas to be treated, degree of correction, and patient and cosmetic surgeon preference. One common technique places incisions in the groin, extending downward and wrapping around the back of the thigh. The underlying tissue will be reshaped, and tightened, and skin will be reduced and redraped, resulting in more proportionate and smoother body contours. Lifting the entire thigh and buttock will require much longer incisions that start in the buttock crease, extend around the groin, come up over the side of the abdomen and reach over the flank and across the back. Deep support sutures within underlying tissues help to form the newly shaped contours. Sutures close the skin incisions. How long will it take? About two to four hours. How much will it hurt? Anesthesia prevents pain during plastic surgery. Pain following a thigh and buttock lift is typically controlled by oral pain medications. The first day of surgery, you may require injections or intravenous pain medications, but usually pain pills relieve discomfort after that. Average hospital stay. Buttock and thigh lift surgery is performed on an outpatient basis. Depending on the size of your lift, you may leave the hospital on the same day of the procedure or spend one or two nights in a skilled nursing facility or hospital. Postprocedure care. After your thigh and buttock lift: Dressing or bandages may be applied to your incisions Compression garments may be placed to control swelling, support the suspended tissues, and smooth and flatten the skin One or more small, thin tubes may be temporarily placed under the skin to drain excess fluid or blood that may collect Sutures in the incision closure may require removal Activity is determined by the progress of the healing of the incision You will be given specific instructions that may include: How to care for the surgical site(s) following surgery Medications to apply or take orally to aid healing and reduce the risk of infection Specific concerns to look for at the surgical site or in your general health When to follow up with your plastic surgeon Call your doctor After arriving home, contact your cosmetic surgeon if any of the following occur: Signs of infection, including fever and chills Persistent high temperature Redness, swelling, increasing pain, bleeding or discharge from the incision site Opening of any part of the incision Any other problem or concern In case of an emergency, call 911. Possible complications. Complications are rare but no plastic surgery procedure is completely free of risk. The shape and length of the incision will be determined by the degree of the necessary tummy tuck correction. You may be asked to stop taking some medicines or herbal supplements up to one week before the procedure, like: Aspirin or other anti-inflammatory drugs Blood thinners, such as clopidogrel (Plavix) or warfarin (Coumadin) Ginkgo biloba or others Anesthesia. You may have general or local anesthesia. You will be asleep with general anesthesia. Local will numb the area. In this case, you may also be given a sedation medicine. It will help you relax. The type of anesthesia used will depend on your procedure. Description of the procedure. There are three main components of nose surgery: Lifting the skin of the nose from the bone and cartilage. The nose will be injected with a numbing medication and epinephrine. The epinephrine will prevent excessive bleeding. An incision will be made either inside the nostril, or outside the nostril, across the ridge between the nostrils. The skin will then be lifted off the cartilage of the nose. Remodeling bone and cartilage. Depending on the desired outcome, some nasal bone may be removed, fractured and reset to a new shape. Cartilage may also be trimmed. Other techniques involve placement of tissue grafts (bone, cartilage or mucosa) from the patient or a donor, or a synthetic graft. They will be used to help remodel the shape of the nose. For example, the tip of the nose may be narrowed or raised, the slope of the nose may be reduced or increased. The shape or size of the nostrils may also be changed. Redraping skin over the new base. Once the procedure is finished, the incisions will be closed. The skin will be redraped over the new bone structure. The skin will be tightly taped to keep it in place. A protective metal splint will then be applied on the outside of the nose. It will help to maintain the positioning during healing. Either soft plastic splints or gauze packing coated with petroleum jelly will be used as nasal packs. They may also be inserted into the nostrils as support during healing. How long will it take? About two to four hours. How much will it hurt? Anesthesia prevents pain during the rhinoplasty surgery. There will be some pain for about one to four weeks after the nose job procedure. You will be given pain medication to relieve this pain. Average hospital stay. You will most likely go home on the same day. Postprocedure care. To help ensure a smooth recovery: For the first few days after nose surgery, rest in bed with your head elevated to reduce swelling. Apply ice packs to your nose during the first two days. This will help to relieve discomfort. Take prescription or over-the-counter pain medications as directed by your plastic surgeon. Do not blow your nose at all during the first week. Do not blow your nose forcefully for one month. Protect your nose from any injury for eight weeks. Do not wear glasses. Be careful when washing your face. Avoid vigorous exercise for at least three weeks. Avoid contact sports for six months. Be sure to follow your doctor’s instructions. Stitches will be removed after the first week. Nasal packing will be removed after one to two days. The splint is worn for one to two weeks or more. Numbness, swelling or bruising of the surgical area and around the eyes should be expected. Expect swelling and bruising to worsen over the first several days after the rhinoplasty surgery. You can also expect some bleeding from the nose, headache and a feeling of nasal congestion. Once the swelling and bruising have diminished, expect full healing in about three to four weeks. Possible complications. Complications are rare but no plastic surgery procedure is completely free of risk. If you are planning to have a nose job, your cosmetic surgeon will review a list of possible complications, which may include: Poor healing of the skin Unsatisfactory cosmetic result Increased risk of nosebleeds Infection Reaction to anesthetic Puncture of septum or collapse of nose bridge (relatively rare)
